First Visit
When you come in for you first visit with us, there are several things to remember to bring:
- Your insurance card and information
- Any medical/dental cards or plans that you are enrolled in
- Photo ID
- A list of all your current medications
We also ask that you arrive 15 minutes before your appointment to fill out any paperwork we have for you. We want your experience with us to move along as seamlessly as possible.
